How can I submit the signed Elevate Health Form?

It is simple to submit your Elevate Health Form (physician/doctor form) to your EHA Wellness account. Follow the steps below. 

Through the App:

  1. Log in to your Abhaya account

  2. Tap View Rewards on the Your Rewards tile on your dashboard

  3. Tap Submit Form on the Elevate Health Form section

  4. Tap Select File or Take Photo and select the image/PDF file of your form or take a photo

  5. Tap Submit Form after the progress bar fills up to 100%

  6. Your Elevate Health Form will be submitted for review. Please allow 24-48 for review

Through the Website:

  1. Log in to your EHA Wellness account using this link

  2. Click on View Reward on the Reward Center tile on your dashboard

  3. The Reward Center will appear on the right pane. Click on Submit Form on the Elevate Health Form section

  4. Click Choose File and select the image/PDF file of your form

  5. Click on Submit Form after the progress bar fills up to 100%

  6. Your Elevate form will be submitted for review. Please allow 24-48 for review

Note: Please convert the signed form to PDF format before uploading it to the portal.
