How can I download the Elevate Health Form to get it signed by my doctor?

It is simple to download your Elevate Health Form (doctor/physician/healthcare provider form) from your EHA Wellness account.

Through the App:

  1.  Log in to your EHA Wellness account on the Abhaya Well app.

  2. Tap View Rewards on the Your Rewards tile on your dashboard.

  3. Tap Download Form in the Elevate Health Form section.

  4. The Elevate form will open in your mobile browser, and you can download it.

Through the Website:

  1. Log in to your EHA Wellness account using this link

  2. Click on View Reward on the Reward Center tile on your dashboard

  3. The Reward Center will appear on the right pane. Click on Download Form in the Elevate Health Form section

Note: You must complete your PHA prior to downloading the form. Otherwise, an error message will pop up.
