How can I get my Elevate Health Form form signed?

To complete your Elevate Health Form, you must review your Personal Health Assessment (PHA) report with your healthcare provider. We recommend completing the following steps:

  1. Complete your PHA online. This is required before downloading your Elevate form. 

  2. Download and print the Elevate form from the Reward Center.

  3. Print your PHA report.

  4. Take both your PHA report and the blank Elevate form to your healthcare provider. 

  5. Your healthcare provider will fill in the required information on the form and sign it.

  6. Upload your completed form to your EHA Wellness account for review and approval.

Note: You must complete your PHA before downloading/uploading your Elevate form. Please note that your form will be rejected if it does not include a healthcare provider's signature. In this case, you will have to re-upload a new completed form.
