How do I access my PHA report?

Through the App:

  1. Tap Assessment on the bottom navigation menu.

  2. Tap View Report to view the detailed PHA report.

Through Web:

  1. Login to your EHA Wellness account using this link.

  2. Click PHA on the left navigation menu.

  3. Click View Report to view the detailed PHA report.

Note: You can also download the PHA report by clicking Print.
